Student Injury Report

This Google Form is to be completed by school staff any time a student is injured during the school day, whether on campus or during a school-supervised activity. Accurate and timely documentation helps ensure student safety, provides a clear record for families, and supports district reporting requirements.

Please complete all sections of the form on the same day of the injury, and as soon as possible after the incident. Include as much detail as possible. This report will be reviewed by the school office and shared with district administration if needed.
