Medication Information for Parents

Home is the best place for your child to receive medication. If possible, spacing of medication dosages should be arranged to allow for home administration. The Central Point School District follows state guidelines for medication administration. Please review the information below to ensure safe, accurate medication administration.

Medication Forms Must Be Updated Each School Year

Prescription Medications

Medication that will be administered to a student during school hours by school personnel must be provided by the student’s parent. To make a written medication request, the CPSD6 Medication Authorization Form must be completed and returned to the school office. If the circumstances are questionable, the school employee reserves the right to deny the parent's request.

Medication must not be expired.
Must be prepared and labeled by a U.S. pharmacist.
Label or prescriber's orders must have child's name, name of medication, route of administration, dose amount to be given at school, frequency/time of administration, and physician's name.
Prescription medication labels for limited time periods may require a new prescription label or documentation from the physician to be extended beyond the originally prescribed time frame. Examples include medication on the controlled substance schedule which are typically valid for thirty days.
Medication must be transported to and from school by the parent/guardian.
Must be in a prescription bottle.
Most pharmacists will provide two containers for prescription medication at home and school.
All prescription medication, must be kept in the nurse’s office or school office and administered by the nurse or another authorized district employee.
All medications must be picked up at the end of each school year or they will be discarded.

Self-Administered Prescription Medication

All prescription medication, must be kept in the nurse’s office or school office and administered by the nurse or another authorized district employee, unless the student is authorized to possess his or her own medication because of asthma or a severe allergy or as otherwise allowed by law.
Students are allowed to self-administer prescription medications, carry emergency medications for immediate access IF permission is obtained and the Self-Administration Medication Authorization form is completed and on file in school office.

Nonprescription Medications

Must be in original container (no baggies, foil, etc.) and labeled with child’s name.
Physician order needed only when the parent’s instruction for administration contradicts the safe dosing instruction on the bottle/container.
Students are allowed to self-administer non-prescription medications IF permission is obtained and Self-Administration Medication Authorization form is completed and on file in the school office.

Allergy Action Plan
Snack Guidelines for Parents

Anaphylaxis is a sudden, life threatening, severe allergic reaction. The most dangerous symptoms include breathing difficulties, a drop in blood pressure or shock. While CPSD6 cannot guarantee an allergy-free environment at school, the district is committed to doing everything possible to ensure the safety of every child who has allergies. To ensure student safety, Parents/Guardians are required to bring the medication and necessary forms to the school office and conference with the District Nurse.

The student must have a current Allergy Action plan on file with the District Nurse and school office.

Asthma

Asthma is a chronic, long-term lung disease that inflames and narrows the airways. Students with
asthma may experience wheezing, chest tightness, shortness of breath, and coughing. Sometimes asthma symptoms are mild and go away on their own or after minimal treatment with asthma medicine. Other times, you feel fine, but you still have the disease and it can flare up at any time.

Therefore, students with asthma should work with the District Nurse to create an Asthma Action Plan and signed by the parent. This will allow the District Nurse and School staff to monitor and manage the student’s asthma disease in accordance with the plan.

Seizure Care

Seizures are temporary changes in behavior caused by problems with the electrical and chemical activity of the brain. Seizures may look and feel different from one person to the next and what happens during a seizure depends on the area of brain affected. Epilepsy may also be called a “seizure disorder.” Seizures can be unpredictable and it is hard to tell how long they will last or what will happen during them. Some may start with minor symptoms, and then lead to a fall or a loss of consciousness; other seizures may end in just a few seconds. Therefore students with a history of seizures should contact the District Nurse to create an Seizure Action Plan. The Seizure Action Plan is signed by the parent and District Nurse. If your child requires medication as an emergency intervention to control or stop a seizure, medication orders must be completed by a provider in addition to the Seizure Action Plan. This will allow the District Nurse and Campus Staff to monitor and manage the student’s seizure activity in accordance with your physician’s orders. Student’s who require medication at school to help control seizure activity must submit the CPSD Medication Authorization Form.

Vision Resources

Vision screening is a simple test that indicates whether a child may or may not have a vision problem. This is important, as it’s estimated 80% of learning is visual. Catching a vision problem early can have a profound effect on a child’s life.

In CPSD6, students in Kindergarten, 1^st, 2^nd, 3^rd, 4th 5^th grades receive annual vision screening. If you need help finding an eye doctor or getting an exam and/or glasses, contact a District Nurse to access the multiple resources they have available regardless of insurance or income level. If students do not pass the screening, they should be taken to see an eye doctor, such as an optometrist. If you need help finding an eye doctor or need a voucher for a free appointment, contact your District Nurse.
